Image/Tube of choice. I am using the fabby work of Zindy Zone which may be purchased for usage here. She also has some free to use tubes here.

Template 135 by Missy
Inkster Font found at www.fonts.com
Eye Candy 5 Impact (optional pay filter) used for gradient glow effect
Get comfy and remember to save often!
1. Open the template.
Shift+D to Duplicate.
Close out the original.
2. Delete the by Missy layer
3. Resize your canvas to give you more room to work in.
Image|Canvas size-->change the Width to 800 and the Height to 500 with Pixels selected
4. Floodfill the transparent background area you just created with white.
5. Make the heart 3 layer active.
Layers|Merge|Merge Down
Make the hert 2 layer active.
Layers|Merge|Merge Down
You are now left with a layer called heart 1 where all the hearts are merged together.
6. Make the raster 1 layer active (rounded rectangle).
Click in the middle of it with the magic wand.
Selections|Float
Open T_Paper7
Shift+D to Duplicate. Close the original.
Resize to 20%.
Copy|Paste as a new layer above the floating selection.
Selections|Defloat
Selections|Invert
Make the paper layer active
Edit|Clear
Selections|Select None
7. Make your heart1 layer active.
Click in the middle of each set with your magic wand tool. Make sure your
mode is set to Add so that both portions of the hearts will stay selected.
Selections|Float
Copy|Paste the image you are using as a new layer above the floating selection.
Position your image so that the portions of it you want to keep are inside the marching ants.
Selections|Defloat
Selections|Invert
Make the image layer active
Edit|Clear
Selections|Select None
8. Make your brackets layer active.
Using the same process you used in step 7 add a paper that cooridinate with your image.
I used T_Paper5 resized to 20%.
*You can use a different method but using the float|defloat feature is one way to keep your rounded edges from being jagged.*
9. Delete the template layers, leaving only your tag you have thus far.
10. Make the image|heart layer active.
Add a drop shadow of
V->1, H->1, O->50, B->12 Color Black
Change the blend mode to this layer to hard light.
11. Make the rounded paper rectangle layer active.
Add a gradient glow using the Eye Candy 5 Impact|Gradient Filter with these settings.
12. Use embellishments of choice to decorate the brackets. See my tag for reference.
Go back and add drop shadows to any layers that you want to add depth to.
13. Add a quote of choice to your tag or copy the one I used.
14. Add ©info and your name.
15. Crop your tag to remove the excess canvas.
16. Merge all layers and save as a jpeg OR if you want a transparent background delete the white layer, merge visible layers and save as a png.
